
Male donor eligibility qualification is required when sperm is to be used either with a known recipient or for use in creating an embryo for implantation in a gestational carrier/surrogate who will carry the child.
The service includes consultation with the male donor, an informed consenting process, processing of the initial specimen, physical examination, and a comprehensive semen analysis. (including strict morphology and biochemical analyses when performed simultaneously with cryopreservation).
Blood testing is not included in this Fee. It is performed at an FDA-approved laboratory at the time of specimen cryopreservation and six months after specimen storage.
